The Home and Community-Based Services (HCBS) programme, delivered in partnership with the Department of Health and Wellness, is dedicated to strengthening access to quality primary health care within our communities. Primary health care addresses the most pressing health challenges faced by individuals and families by offering promotive, preventive, curative, and rehabilitative services.
Through our wellness sites and community outreach, we place strong emphasis on prevention and early intervention, raising awareness that enables healthier choices and timely action. Our overarching objective is to support, uplift, and empower communities by creating opportunities for education, wellness promotion, and skills training.
Together with our partners and the communities we serve, we strive to build a foundation of health and dignity that promotes resilience, equity, and sustainable well-being.
Through our dedicated team of Professional Nurses, Enrolled Nurses, and Community Health Care Workers, we deliver a wide range of interventions designed to strengthen health and well-being at every stage of life. These services support individuals and families to maintain their health, prevent illness and accidents, manage minor ailments and long-term conditions, and recover after periods of acute illness or hospitalisation.
By working together, our team provides not only care but also encouragement and guidance, ensuring that communities have the knowledge, resources, and compassionate support needed to live healthier and more resilient lives.

Our teams provide community-oriented primary health care across the identified areas, ensuring that care is accessible where people live, work, study, and grow. Home-based care is offered directly at residences, workplaces, tertiary institutions, and schools, extending support beyond traditional health facilities.
Many of our patients are referred by hospitals and healthcare facilities, allowing us to continue care in the community and within the home. Through household assessments, we identify families and individuals at risk, reduce potential health challenges, and promote proactive measures for holistic wellness.
In this way, we bring compassionate, patient-centred care closer to the community, helping to build healthier and more resilient households.
