The aim of our Home Community Based Services program, in partnership with Department of Health, is to provide and promote primary health care services. Primary health care addresses the main health problems in the community, providing promotive, preventive, curative, and rehabilitative services. To achieve this, we provide prevention and early intervention awareness. Our objective is to support and uplift by creating a platform to empower the communities through education, wellness promotion and training.
Through a team of trained Professional Nurses, Enrolled Nurses and Community Health Care Workers, we provide an array of interventions that support the actions people take to maintain health and well-being; prevent illness and accidents; care for minor ailments and long-term conditions; and recover from periods of acute illness and hospitalisation.

Our teams offer community orientated primary health care in the areas identified above. Home Based care is provided at your residence, place of work, tertiary institution, and schools. These services are rendered to patients who are referred to us by the hospitals and healthcare facilities. Through household assessments we aim to reduce health risks by identifying households in need and encourage proactive measures for holistic wellness.
