Home Community Base Service

The vision of Tygerberg Hospice Home Community Base Care (HCBC) program is to provide health services by formal and informal Community Health Workers in order to promote, restore and maintain a person’s maximum level of comfort, function and health including care towards a dignified death.

We aim at providing an array of interventions that support the actions people take to maintain health and well-being; prevent illness and accidents; care for minor ailments and long term conditions; and recover from periods of acute illness and hospitalisation.

Our goal is to prevent unplanned visits to health services through high-quality and appropriate care that helps the user population to maintain their independence and achieve the best possible quality of life.

Admission Criteria:

  • Compromised functional status requiring supported self-care
  • Requires treatment adherence & support
  • Needs screening, health education and support to stay healthy, safe and free from abuse and neglect
  • Requires end of life care
  • Psychosocial support is help with social, emotional, psychological and practical needs (i.e. the way we take care of one another) and building relationships of respect and acceptance where the dignity of people is strengthened
  • Minor health problems/ ailments are conditions which, with simple actions, patients could handle themselves (for example colds, flu, intestinal worm in children, constipation, allergies, etc.)


  • To promote health and prevent illnesses and disability
  • To mobilise around community needs
  • To identify households’ health needs by completing a standard assessment
  • To provide psychosocial support to households
  • To provide education and advice on minor health problems
  • To support health promotion programmes in schools and ECD centres
  • To support continuum of care (for example, by signposting people to information about local health and care services and how to access them)
